Introduction

There will be increased efficiency in the provision of care services to patients particularly discharge services. Further, better teamwork among health workers and patients a well as increased patients satisfaction also increases.

Communicating the impact of the program such as reduced rehospitalization and efficient healthcare delivery will help the decision makers consider investing their funds and time in the project . The presenter has to show qualitative and quantitative evidence to make the corporate consider the project.
